The square collaborative practice model and education project has been developed through the efforts of a great many people, including staff from many Divisions of General Practice, the SA Divisions of General Practice Inc, SA Department of Health, SA Mental Health Services, Australian Government Department of Health and Ageing, Relationships Australia (SA) and carers and consumers.
